1) Create or login to your Teen Ink account

2) Fill out this form

3) Select "Submit Form" and you're done!

Deadline: April 11, 2022


All submissions to the form will be uploaded to your Teen Ink account and published online

- Only middle school/junior high or high school educators eligible

- Nominator must complete nomination form 

- Essay must be typed or uploaded into form

- Must be age 13-19 to nominate someone

- Nominator must have a Teen Ink account

- No inappropriate content

- Nomination category must be selected
                  - All nominations will be considered for the overall winner regardless of category
                  - Categories are:
                    Math, Science, English, History/Social Studies, Creative Arts, Facilitators, and School Support

- Give examples with specific details!

- Essay must be around 500 words
