This poem is about never being able to trust that those who care for you truly do want what's best for you. If you've been hurt once, you are constanly weary that it will happen again. Many who are damaged cannot trust those who so dearly want them to be okay. The "battle" is between self-hating thoughts and the support of others; which words are true?
