Am I slowly disappearing?
Slowly becoming true oblivion?
Will I fade from all memories?
Or will I just fade into the darkness?

Short and sweet. I love poems like this. Just be sure you inject enough pure emotion into the words. I think you did a fairly good job of that here, but it lacked a little bit of punch. This is definitely, to me, one of the hardest ways to write a poem, but I think you pulled it off.
